You might be surprised. It is definitely in the plans of the hospital  
I work at. The entire, very large hospital complex in downtown Miami  
is now surrounded with a high, spike-tipped (yet decorative ;-) fence  
to keep out victims in the event of overwhelming emergency. The  
perimeter has new "landscaping berms" carefully sited to provide law  
enforcement protection as they cover the fence. The disaster plan  
includes methods to ferry key staff into the complex in the event it  
is surrounded by a hostile mob. After this event, they will probably  
add barbed wire...
